We face a global antimicrobial resistance crisis. In 2019, there were 1.27 million deaths attributed to AMR. No new classes of antibiotics (AB) have been developed for decades. Here are some quick thoughts on why new development is slow, and what we can do about it.

Discovery process is lengthy and fraught with investigative dead ends. Finding a new AB needs lots of time, people, and MONEY. (Half the cost is in the pre-clinical phase.) But leaving aside scientific and regulatory barriers...

Any new AB must be closely stewarded, meaning its use will be discouraged. This is a financial DISINCENTIVE for any commercial entity to want to develop one.

Solutions include a "market entry reward", wherein small biotech firms are given lump sum payments my government for bringing new AB to market.

Guaranteed purchase in the form of government stockpiling. This creates an emergency supply in case of bacterial pandemic, and helps to offset financial disincentive against new AB development.

Anyone else notice how the provincial government seems allergic to the words "family doctor"? They will only say "primary caregiver", which includes NPs and pharmacists. The reshaping of reality begins with language choices. Watch for it.
