@ned Sensible people would, I think, try very hard to minimize risks. For example: If we can't surface after the dive, we'll die. So how many ways can we figure out to get back to the surface if the first method fails, and the first backup method fails, etc. Twenty methods is probably too expensive and not practical, but I would personally want at least three independent ways of coming up. Four would be better. If five days of oxygen is normal, how can we extend that to eight? And so on. Taking stupid risks with clients' lives is not "breaking the rules"--it's idiocy.

Basic lack of understanding about the difference between redundant systems (and contingencies) and duplication.
I agree in health care there is a lot of duplication (safety clutter) that doesn’t add to safety or create redundancy but often detracts from it.
However, safety is not waste.
