Humans are good at a lot of things, but they're not good at eternal, perfect vigilance. Writing code is hard, but performing code-review (where you check someone else's code for errors) is much harder - and it gets even harder if the code you're reviewing is usually fine, because this requires that you maintain your vigilance for something that only occurs at rare and unpredictable intervals:
