I think it might be an issue arising from a struggle to distinguish self-identity at all. Related problems manifest in autistic individuals who cannot code switch (or, in non-retarded language, "behave differently according to social context") and end up perceiving different behavior sets as entirely separate personalities that co-exist with each other, to which they sometimes give separate names/sexes and narrate as "headmates".
