My last job before I retired was a civil service program manager in an aircraft manufacturing plant. Quality escapes happened like the ones Boeing is experiencing now. The manufacturing company did away with their QA inspectors at the inbound parts department to save money. The aircraft was in its 40th year of production & there were still quality issues/escapes. Usually a new guy not following procedures was the root cause. The company fixed the escape in the field at no cost to the customer.
