Apparently the little tab on one of the washers was not locked into the tab slot, so it was just insecure enough to slowly work itself out, so the wheel fell into the brake disk, slid out and I went over the handles on one fork.
I fell slow and landed flat. Helmet saved my ass a trip to the hospital as they do.
Kinda pissed the instructions don't mention to pay attention to that tab's direction, and my hubris for not cross referencing other assemblies putting the wheel on. -_-
