I love genuine questions and people putting in the effort to love and understand each other better. If you come at me just wanting to argue I’m going to troll you back. FAFO.

Please please please go on www.stuffthatworks.health and rate ALL of the treatments listed for any disorders you have. It’s a patient-reprted knowledge base for both patients to help each other and so providers can gain a more in-depth perspective of patient experience. The experiences of someone who has tried as many treatments as you will be INVALUABLE to that community. Edit: fixed the link.

Apytele,

I pretend to be an old demented lady whose granddaughter keeps hiding my wallet because she “thinks I’m too nice.” I “take the phone with me” while I “look through” the couch cushions, the fridge, the garage, and more. Every time they’re about to hang up I think up another place I could’ve left it or my granddaughter could have hidden it. When they get desperate I “find it”, but all of the cards are expired.

Apytele,

It’s also completely undetectable to a reproductively abusive partner (clinical term for baby trapping). This allows the person to get their ducks in a row to leave the abusive relationship safely and for good, without alerting the abusive partner OR being trapped into being in some kind of contact for 18 years. By contrast, the closest women have is the depo-provera shot which only lasts 3 months and is arguably the harshest of all hormonal birth control options. RISUG is one of if not the best birth control method we’ve invented to-date, and we’re sleeping on it.

Solution: make a social media with one crappy low-res picture of your face, one crappy low-res full body shot of you engaging in a hobby you can talk intelligently about and a couple stock photos of sunsets and other nature pics. Caption all of the nature pics with some variation of “I’m practicing gratitude today! I’m so grateful to the universe for waking up this morning!” You have now revealed nothing they didn’t know already, and have a “social media presence” that appears positive and emotionally well-adjusted to normies. You’re welcome.

Apytele,

I said what normies will perceive not what is actually true. Although I will mention that gratitude is actually clinically proven to improve quality of life, but people also tend to frequently confuse it with toxic positivity. The example I have above is technically toxic positivity, but you’re doing little enough of it that the cost benefit analysis works out.

Apytele,

There’s too many people out there cheating on each other and telling their side piece that they just don’t have one …then they turn out to be married and just don’t want to show you so you don’t see the pictures of them with their spouse. Men AND women as I understand it.

Apytele,

I’ve had to learn this too. A lot of people really could use a big heaping spoonful of “stop being right about shit, start getting shit done instead.”

I mean, I am an alcoholic, so. I’m doing better though. I’m trying to only drink when offered one (from a trusted source) and that’s been going ok so far. And since all of you are so very concerned, I have received over 300 hours of formal, licensed therapy which I have used to inform my selection of supporters and the mechanisms they use to support me. If you are licensed in one of the forms of therapy that have been invented in the past year and therefore one of the only ones I haven’t tried yet, please PM me your number because I am very interested.

Apytele,

Every single one of these concerns has already been addressed (for instance I have already received over 300 hours of formal therapy to date). Thank you for your time.

Yeah that’s not what S.M.A.R.T. therapy said, and it’s both evidence-based and grounded in modern therapeutic practices. I’m sorry your only experience with addiction therapy has been the archaic hyperreligiosity of AA and that you were never taught any kind of coping skills for urges other than increased spirituality, which honestly explains a lot about why people who have only received that as their therapy are never capable of learning moderation, and why it doesn’t work AT ALL for behaviorally based addictions like food or sex (how do you stop eating?). Anyway I’m going to keep drinking the single beverage my life partner brings me at parties like I have for a few years now. I very well might need to change my strategy in a few years. Fortunately that is something else I learned how to do in S.M.A.R.T…

Apytele,

Honestly my first thought looking at that is literally just oh god how much would that bill be. The heights of alcoholism get really expensive. In multiple ways!

Apytele,

yes, the one based on christian puritanism is doing muuuch better. And I’ve also done CBT, DBT, WRAP, and yes, even AA because there’s something to learn from everyone. DBT is the one I’m professionally trained in, though.

What are your favorite fonts for technical reports?

I work at a consulting engineering firm and write a lot of reports that are read by the public. I have an opportunity to recommend a different font for all of our written documents and am looking for something more modern/fresh than Times New Roman. Also open to recommendations for purpose specific communities about...

Apytele,

Calibri has always looked nicest to me, but I don’t write technical reports.

What's a decently fancy coffee that's still affordable and will be well received even by non-coffee snobs?

I have been making the occasional coworker a cup of aeropressed coffee with good reviews thus far. Being that it is shift work, most of my coworkers are used to drinking the reduced syrup of a pot that’s been left on the burner too long (one coworker thanked me for leaving it for her!). Many don’t even know coffee doesn’t...

Apytele,

The message to me was just that sometimes you have to sing and dance to entertain the people who have power over you. That doesn’t make the situation right, but I’ve gotten a lot more done since that realization.

Most Americans with mental health needs don't get treatment, report finds (www.npr.org)

Roughly two-thirds of Americans with a diagnosed mental health condition were unable to access treatment in 2021, though they had health insurance. And only a third of insured people who visited an emergency department or hospital during a mental health crisis, received follow-up care within a month of being discharged....

I’m an inpatient psych nurse. We do need more facilities and clinicians, but the muuuch bigger issue is affordable housing. I would say about 33% of our unit census at any one time, especially in the winter, often boils down to people staying for what I like to call “3H1C” treatment, or “three hots and a cot.” They’re homeless and come to the Emergency Room and say they’re suicidal (the good ones fake or exaggerate psychosis) so they won’t freeze to death or otherwise die of exposure. I don’t blame the doctors for not taking any chances, and I don’t blame the patients for lying. I also honestly just don’t want to live in a world or work in a system where I have to try and figure out if someone is lying about wanting to die.

Honestly I don’t even really want more homeless shelters; we need to make housing affordable again. We can’t be keeping people in shelters, especially not when they also have mental health concerns, and there’s still a pretty big overlap in those populations. Just because they’re not suicidal and don’t need to be cared for at my very invasive and expensive level of care, doesn’t mean they’re mentally healthy enough to not shank their bunkie because they have PTSD from some kind of institutional or homeless experience. Some of these people jump up out of bed in a full rage if you’re not reeeeaaaallly careful about how you do it, imagine them sleeping in bunk beds! The only way to keep people safely packed in like that is an immense amount of supervision. It’s honestly probably cheaper to just give them more personal space. Many of them will still need significant assistance to safely live independently due to the psychological effects of chronic institutionalization, but coming by once a week to teach them how to cook or do their own laundry is a much cheaper, less invasive, and more humanizing approach.

Edits: sorry I just keep adding shit and fine tuning I’m way too ADD.

Apytele,

Honestly (not that this is right) but a lot of that is that there’s a lot of pill-pushing in the rest of medicine to account for our own labor shortages. Pills are the easy way out for us with our insane patient loads and acuities, and they’re the easy way out for the patient eating fast food on their way between two jobs who don’t know what OSHA is that they don’t get breaks at and 9 total hours of sleep THIS WEEK because they also have three kids.

what is a skill you wish you had, and why?

Ok, I might as well go first: I wish I could draw. Not at the level where I could make photorealistic portraits, but I’ve always been envious of those who are able to scetch something together in a few minutes that perfectly captures what they want to convey. Sometimes words aren’t enough to express what I want to say, and...

Remembering names and faces. I’ve got a huuuuuuge variety of valuable knowledge and skills and I’m ALWAYS getting tripped up on people’s names. I’ve jumped between a physical aggressor and their victim before (I work in that kind of field) for coworkers whose names I cannot remember. What the fuck is wrong with me? My brain isn’t even any bigger than anyone else’s it’s just stuffed with knowing how to spell and pronounce N-acetylcysteine instead of anything actually useful.

Apytele,

Take pictures of yourself; it will help illustrate the point much better. You could also make them a short video or pay for stock photos. Most stock photography is in one of a few very specific styles because they best show objects and actions, so they should be relatively visually consistent but you could also use gimp to help keep the colors consistent. Gimp does also have some cartoon filters, but they take a lot of editing to look good. Might still be easier than trying to draw it yourself or commission something.

Apytele,

yes in that it’s a diuretic, no in that it’s not actually good for your kidneys. But now that you mention it…

Apytele,

Oh, bless! I wish you as much health as you can get!

Apytele,

bruh you need to train more on the pushups and pullups and spend less time in your routine on practicing fuck ups!
