How many industrial incidents could be prevented if proper procedures were followed & reports were not ignored?

Former Goodall rubber plant in NJ burning down, taking multiple businesses with it. No exact cause has been made public, but there seems to have been masses of improperly stored flammable objects on pallets literally all over outside the building.

Nearby roads closed & temporary power lines installed to get nearby residents back in their homes.

Hunkered into 2 rooms of this house that are connected via open door. HEPA filter air purifier in each room. Cats, their food, water & potty in there too. Then here is me out here making dinner for myself with my 3M 6000 series elastomeric half face mask + P100 filters. AQI is over 300. Visible plumes of smoke billowing through the neighbourhood separate from the overall dense haze. Yes, you can see the smoke moving.
