With the recent news that the r/blind community has migrated to a lemmy instance, I thought now would be a good time to post a quick PSA on image descriptions....
Apart from adding alt text (which someone in another comment has already explained how), I want to add other ways to help Blind and visually impaired people and other people that require the use of screen readers and text to speech apps (like myself)
When using tags with more than one word, always write the first letter of each word in uppercase so the screen readers and text-to-speech apps can read them as separate words

Avoid special fonts as they cannot be detected by screen readers and TTS software. Most of the time the letters are actually phonetic or mathematical symbols and the software interpret them as such.

Others have answered the crux of your questions, which is that it's basically donations... either from the admins by providing free access to their server, or by the community through Patreon or whatever.
But to put into context how much money we're talking about...
A server to host 1k active users and 5k-10k registered users, you're talking about a 4cpu-8cpu box costing less than $20/mo. Plenty of nerds with decent jobs in wealthy countries are willing to write that off as a donation. This covers 99% of the <1k Lemmy servers in the world.
The 10 biggest Lemmy servers still only have hosting costs of $50-$300/mo. That's not nothing, but there are probably 10 wealthy nerds in the world willing to write that much off each month. And those costs can be offset through community donations. These servers support 10k-40k registered users, it doesn't take a ton of donations to cover that modest expense serving that many people.
Now, if you count admin/mod time and expertise, of course... those costs would be huge. But those people either volunteer or get a bit of money from non-profits. But the hardware costs are modest.
Currently, on the main instance, people have created 40191 accounts (+214 marked as deleted). I don't know how many are active because I don't monitor it, but once again, I greet all of you here :) In recent days, the traffic on the website has been overwhelming. It's definitely too much for the basic docker-compose setup,...
The biggest reason I'm here, is Ernest. Getting in when a project is just getting off the ground, let's users have some say about the direction of things and features. And Ernest is very responsive to the community and asking for feedback. That's exciting, and makes the whole thing feel more of a community.
The Old Fashioned is my favorite drink. It introduced me to whiskey and mixology plus it's simple and versatile. I think it's easy to judge if a bar has good cocktails based solely on their Old Fashioned. What are your opinions, favorite variations, etc?
