It’s takes around 3mm of stubble growth to significantly impact fit of particulate filtering respirators.

“The cats were found dead with no apparent signs of injury and were from a resident population of ≈24 domestic cats that had been fed [unpasteurised] milk from sick cows. Clinical disease in cows on that farm was first noted on March 16; the cats became sick on March 17, and several cats died in a cluster during March 19–20. In total, >50% of the cats at that dairy became ill and died.”

This video demonstrates how well fit-tested respirators work as source control vs no masks vs loose fitting medical masks.
No complicated modelling required.
We can achieve higher levels of risk reduction by having our healthcare workers wear these when looking after vulnerable patients, particularly at times where staff are at higher risk of infection (recently symptomatic, close contact of someone unwell at home, or high community prevalence)
Screenshots in the next post.

So many people going into work symptomatic as they test SARS-CoV-2 negative on RATs where they have the option of working from home, only to make their colleagues sick who go through the same “symptomatic but testing negative routine” until they eventually test positive.
If you can’t work from home, wear a well fitted mask for the sake of your colleagues.

Everyone at home is sick with fevers & coughs except me today. It would be completely irresponsible & a dereliction of duty for me as a close contact for whatever it is my family are carrying to not reduce the potential exposure to my patients by 100 fold by wearing a fit-tested particulate filtering respirator whilst at work.
This is a permanent change to my practice & should be for all healthcare workers going forward.

Imagine being sent into a COVID ward, being given a blue medical mask and being told “this will protect you.” and then suffering long term consequences of COVID-19 limiting your ability to work.

We cannot again be put in the position where limited resources for respiratory protection bias policy decision-making.

Respiratory protection should be an ongoing and increasing component of healthcare OH&S to maintain competency and supply chains.

The assertion that “NPIs were ineffective” during the pandemic flies in the face of overwhelming evidence given the experience of 50% of Australia and 100% of New Zealand that experienced no sustained local transmission during the with early and effective implementation of multiple NPIs.
Context and goals for the region are key considerations when understanding how and when they are effective.

“In fact, one could argue that govt policy didn’t go far enough, with govts seemingly willing to do something that Mr Tucker very much appears to approve of: Sacrificing working class individuals to keep industry working. After all, that was the essence of the GBD, sacrificing the working class so that corporations could keep making money & people like Mr. Tucker were not unduly inconvenienced, damn the cost.”

CDC seeks to update guidance to reflect recognition of aerosol viral spread.

“The US Centers for Disease Control and Prevention (CDC) has asked the Healthcare Infection Control Practices Advisory Committee (HICPAC) to clarify its guideline on isolation precautions to reflect the "widespread recognition" that respiratory viruses spread primarily through aerosols, according to a CDC blog post this week.”

Fascinating paper.

“Infectivity of exhaled SARS-CoV-2 aerosols is sufficient to transmit covid-19 within minutes”

Note that the study uses TCID50 from an immune naive human challenge study.

Ventilation can reduce exposure to respiratory viruses in indoor spaces.

“Steps for cleaner air to prevent the spread of respiratory viruses can be taken any time, especially when:
• Respiratory viruses are causing a lot of illness in your community.
• You or those around you were recently exposed to a respiratory virus, are sick, or are recovering.
• You or the people around you have risk factors for severe illness”

This part of the message should be emphasised.
We need a cultural shift where we take steps to reduce risk to those around us when we’re unable to work from home when sick.
COVID-19 isn’t the only communicable disease for which wearing a #bettermask when :

  1. sick or
  2. a close contact of someone unwell at home

may help reduce transmission.

“Reinfections aren't harmless. As cases continue to rise & more variants arrive on the scene, ID experts are warning that repeat infections could have cumulative, lasting effects. There is some early evidence starting to show that if you had COVID-19, there can be all sorts of problems after getting infected”& reinfected. We are just at the beginning of learning about them.” Dr. Robert Murphy, Havey Institute for Global Health, Northwestern’s Feinberg School of Medicine

Many hospitals I work in have a policy that for a few days after returning from a SARS-CoV2 infection staff need to wear a fit-tested respirator as source control whilst at work.
This recognition of respirators’ utility as source control should be considered to help reduce nosocomial transmission from staff of other pathogens found in the respiratory tract.

@mackayim2022 Totally, and as someone currently in the throes of FluA (-ve antigen test, +ve PCR so missed the Oseltamivir window) despite the seasonal vaccine my body is recognising the failure rate.
