Dr Ruth Ann Crystal: "A new report from the Economist shows the enormous economic costs of Long COVID in the US and in other countries because people with Long are no longer able to work.

In the United States, Long COVID is estimated to cause a whopping $152.6 Billion GDP loss this year due to lost work hours. In the UK, the equivalent of US$15.5 Billion is expected to be lost in 2024 because of .

The report also reports data on expected economic losses from Long COVID in Brazil, France, Japan, Saudi Arabia and Spain. Preventing COVID infections and Long COVID disability should be a priority for all countries for many reasons"

Back when an elderly relative qualified for Medi-Cal (which they tried to deny), they also cut off her food stamps. Saying that because her income was over the gross amount, they had to apply an assets test

Today the worker I spoke to said that was incorrect: she should have qualified (and because of the denial she lost $250 a month in enhanced food stamp assistance)
https://www.kqed.org/news/11943420/your-calfresh-benefits-will-drop-in-april-heres-what-you-can-do

Anyhow....

There is no longer a property limit for most households applying only for CalFresh; however for those applying for CalFresh and CalWORKs, the cash resource limit is $2,750 per family or $4,250 per family if a member is over age 60 or has a disability.

She is 90 years old. She was not applying for CalWorks.

If she is approved for food stamps, she will now get the min help of $35 a month

As America’s health-care costs were rising, were in their crucial early growth stages. Not coincidentally, Komlos has found that this is also when U.S. life expectancies began to falter.

.....there’s an even deeper issue than health care: rising inequality. Exact estimates vary, but sources tend to agree that 1970s were one of the most equitable eras in American history. Then, around 1981, inequality began to rise..."

If you’re given the resources you need to reach your full potential height, you’re also getting the resources you need to succeed in school and beyond.

For the rest of their lives, shorter will bear the physical stamp of the inequality that erupted in their infancy. When we’re trying to explain America’s unluckiest generation, we should consider not just what they’ve become, but how they started out."

Covid virus fragments stay in the body & can combine to form blood clots & inflammation

Dr Ruth Ann Crystal: "A new eye-opening study reported in PNAS shows that when the SARS-CoV-2 virus is destroyed, its “zombie” viral fragments can spontaneously join together with double-stranded RNA, causing proinflammatory supramolecular complexes (XenoAMP-dsRNA) which then can overstimulate the immune system, triggering widespread inflammation including cytokine storm & blood clotting. "Compared to relatively harmless coronaviruses that cause the common cold, the team found that SARS-CoV-2 harbors many more combinations of fragments that can better mimic human immune peptides." This concerns me that SARS-CoV-2 fragments are able to regroup into proinflammatory supramolecular complexes as studies have shown SARS-2 viral particles in the tonsils of children, in the skull’s bone marrow long after a COVID infection has resolved, & in many other places throughout the body."

"New study in Network Open: Testing the limits of fit through extensive re-donning and re-use in medical settings.

Deep dive: 🔥🔥🔥

Healthcare workers were given fit-tested N95 respirators (). Here's what happened.
🔹Same N95 worn for 5 shifts
🔹Shifts typically 10 hrs
🔹Taken off and put back on (doffed/donned) typically 4 times per shift
🔹25% of workers re-donned 7+ times per shift!
🔹Stored in an air-tight bin between shifts (they should have been allowed air flow to dry out..)
🔹39% of users failed to pass a qualitative fit test after shift 1
🔹About 90% failed a fit test after re-use for 5 shifts (typically 20+ re-dons)
🔹Cup-style N95s outperformed others on re-use
🔹Repeat doffing/donning (taking off/putting on) per shift was associated with increased risk of fit failure
🔹Qualitative fit testing overestimates the chances of "bad fit" by 71%

How do I interpret the findings."

"...rather than straps wearing out. Healthcare workers should be trained in re-donning to maximize fit. Anyone in the community can use DIY fit testing to practice how to correctly wear and re-wear a mask and also check how fit holds up after extensive re-use (many hold up well even after re-donning >10 times). Always store masks in places where they can dry out between use, rather than storing in an air-tight container that will retain moisture.

What doesn't the study show?

This was an "effectiveness" study, meaning it examined how well an N95 would hold up under typical use in healthcare given rationing (which was itself mostly unnecessary). It was not an "efficacy" study designed to examine whether an N95 works under ideal circumstances. NIOSH evaluates efficacy, and any approved has established efficacy. ....What the study does..."

"...suggest is the need for more research designed to maximize effectiveness. In other words, how do we help healthcare workers get a good fit even after re-donning?

To me, the answer is PRACTICE... Doff, don, fit-test, repeat. Learn to gain confidence in a fit. Learn where the gaps are. That's what I do. That's why I can go with confidence into just about any setting.

The study did not evaluate . They found cup-style N95s performed best. Anyone passing a fit-test initially in an elasto would have a very high success rate after extensive re-donning and re-use. If people can find an elasto that fits them well, it's often a wise choice.

The study also did not evaluate the fit of surgical or procedure masks. The reason was that none of these could pass fit-testing. They are not designed to protect against COVID. The focus should be on increasing N95 use in healthcare & taking practical steps to optimize benefit. "

The N95 trifold fit failures are..."

"Repeatedly taking off & putting on the same has the risk of stretching out the straps, which can cause fit failure. Putting an N95 on again also carries the risk of doing so sloppily, which can compromise fit. They authors did not evaluate the cause of poor fit, e.g., by helping the worker to readjust their mask to seek good fit. Cup-style N95s may get better fit under ordinary repeat use because they hold shape better. The fit-failure rates should probably be multiplied by .29 (i.e., 1.00 - 0.71) to reflect the oversensitivity of qualitative fit testing.

That would suggest a fit-fail rate of about 11% after a single shift, which is not bad given the length of use (10 hours) & frequent re-donning.

What are the practical implications?

Healthcare workers should be supplied with a new N95 each shift and as needed. Anyone using an N95 should probably replace it after 5-10 times re-donning. This is a conservative approach, since a lot of the mis-fit could have been from poor masking practices..."

"BA.2.86

Yunlong Cao reported that BA.2.86 can evade our neutralizing , even if we had prior XBB.1.5 , meaning that the new fall may not protect well against BA. 2.86. But there is a caveat. It turns out that BA.2.86 can evade antibodies, but it is not as infectious or transmissible as other variants in circulation now.

This is reassuring. We know that antibodies from the new Fall XBB.1.5 vaccine expected out in about 2 weeks, will protect us from EG.5 and FL.1.5.1 variants which are in circulation now in the U.S. and many parts of the world.

Ben Murrell’s lab in Sweden announced that eight out of twelve blood donors with prior XBB.1.5 infection had antibodies that could neutralize BA.2.86. The updated fall booster against XBB.1.5 may provide some protection against BA.2.86. We will have to wait and see"

Go to the hospital, catch Covid

Another 3.8% of cases were considered indeterminate meaning that the patient may have been infected with COVID during their hospital stay or just before they were hospitalized. When community-onset COVID infections were higher, there was an increase of hospital-onset SARS-CoV-2 infections as well"

Poisoning our stem cells

"This week in Cell magazine, there was a review of the important article by Cheong et al. from two weeks ago showing that severe COVID infections can cause IL-6 induced epigenetic “poisoning” of hematopoietic stem cells to make more inflammatory monocytes instead of innate immune cells (B cells, T cells).

The SARS-CoV-2 virus can go to our bone marrow and can reprogram genes in the cells that make all blood cells in the body (hematopoietic stem cells and myeloid progenitor cells). The virus can turn off the genes which instruct bone marrow stem cells to make immune cells like B cells and T cells that could fight the virus, and instead, the virus reprograms our bone marrow stem cells to make highly inflammatory monocytes.

This reprogramming of bone marrow can cause permanent changes which theoretically could lead to the development of chronic conditions such as
