Effects of the design of operating rooms/theatres on safety is vastly underestimated . In this new study we looked at improving the delays for calling for help using an emergency buzzer by making it more visible using a red line from the ceiling to the buzzer.
Installing the line more than halved the number of times calling for help was delayed.

    That “relaxation”, that “workload reduction” you might feel when using or ?

    Guess what that really is?

    Complacency.

    Deadly complacency.

    And it strikes when you least expect it, when the vehicle suddenly encounters a failure and it takes you an additional second or three to regain situational and operational awareness.

    Tesla washes its hands of that though.

    Humans are not machines and human drivers are not homogeneous and it is entirely unclear how long of the duration of this "warning time" needs to be in an infinite number of conditions.

    That is what is called considerations, in part.

    But let us put the "human element" out-of-mind here.

    First off, one really has to "love" the placement of the mobile device charging pads in this vehicle - a vehicle which is unvalidated as it pertains to being equipped with a highly-complex automated driving system, no less.

    The placement of these pads ensure maximum driver inattentiveness to the roadway and to the dynamic driving task.

    This is not a "side issue". The components are an integral part of the human-machine system in terms of a validation process.

    Basic lack of understanding about the difference between redundant systems (and contingencies) and duplication.
    I agree in health care there is a lot of duplication (safety clutter) that doesn’t add to safety or create redundancy but often detracts from it.
    However, safety is not waste.
