While Seabelts carry a similarly high NNT, they dont give me rhabdomyopathy & make my life miserable.

I did what I could. I voted No to Wisconsins bullshit ALEC sponsored proposals.

I raised hell. I helped a true popular uprising block illegal legislative sessions that bypassed state law.

Tom Morello was there. 2010 was when the war started for me (privilege talking right there, while black people, immigrants, and transpersons been assailed from the cradle), though it was probably under Nixon & Reagan for most of you.

I'll concede its not THE intent, nor an overarching goal, and I agree it has way more to do with the domestic situation.

I stand by it being strong man brinksmanship however -- Netanyahu's general actions being to coninuously put pressure on all of the leverage points at his disposal so as to evade justice, and an intermediate objective in such is to put the US in an unwinnable situation, one that WILL cost Biden the election at this current rate.

I'll defer to those with better understanding but the math is the same -- Biden needs to stop arming this guy.

COVID is a more powerful virus with more neurological issues, so it seems the numbers should be the other way around. Out of curiosity, has anyone looked at this?

I know someone on Twitter said they didn't exclude people with dementia, but that seems like it wouldn't explain the numbers appearing backwards...

Selection bias possible. An anosognosia effect possible. Trauma-induced avoidance of healthcare possible. Mixed confounding effect possible.

Matching doesn't mean as much to me; H5N1 definitely was associated with leaky BBB gap junctions and eventual Parkinsonism back in 1918, so it's not like the flu isn't capable of causing neurological sequelae; perhaps the flu is just severe enough to cause a meta-alarm signal from the Anterior Cingulate Cortex and so people sought/seek out evaluations more readily than with COVID, where such an alarm or error/conflict monitoring signal may be more directly assailed by the olfactory bulb to entorhinal cortex retrograde transmission of the virus.

I wouldn't consider follow-ups to be a metric worth monitoring compared to neuropsych testing results as Johns Hopkins did per Dr. Durga Roy.

It could very well a real signal of something, but we know COVID differentially impacts risk of seizure, core symptoms of cognitive impairment, sleep architecture disruption, and at least short-term anxiety/depression. Seeking care in any such case means little to me as there are so many factors that affect this.

For example, short of sudden onset chest pain, calf pain, or shortness of breath, I've all but given up on seeking care and I'm a doctor (reason: system is broken, invalidating, and of little help unless it's an obvious cardiac or cardiovascular effect).

How fucked up is that?

I do take issue with the notion of a lower incidence and note their language parsing here:

"A lower diagnosis incidence." (actual language: incident neurologic diagnosis)

No shit, if they dont seek care, you won't see them.

That does NOT at all mean less neurological sequelae. See: primary literature showing exactly the opposite trends.

To me, this is a case of a large N being used to drive an agenda to downplay known pathophysiological insult COVID causes both directly (less dominant) & indirectly (particularly BBB gap junction leakyness & microglial dysfunction) until proven otherwise if I saw this in a journal club.

If COVID assails parts of the brain involved in meta-awareness of one's faculties, OR if there is a social shame factor to COVID, OR if there's a culture of denial fostered in the first place that is qualitatively different from any subculture surrounding the flu, then all bets are off and this is a poor metric.

Niacin is a case study in the misuse of intermediate indicators to indicate something it should not.

  1. Studies showed niacin appeared to lower cholesterol levels
  2. It became common practice to use niacin to do so. "Safe," "supplemental". for good "heart health"
  3. The intermediate indicator here, "Cholesterol" level, was a substitute for terminal indicators -- all cause cardiovascular mortality & the ultimate terminal indicator -- death.
  4. We found out years ago that niacin actually is a terrible idea to use, as it may lower cholesterol, but generate other downstream indicators of risk & harm:

"Niacin, also known as vitamin B3, is a water-soluble B vitamin found in meat, fish, nuts, legumes, brown rice and fortified cereals...Being water soluble, excess levels of niacin are usually excreted in our urine. But new research from the Lerner Research Institute in Cleveland, Ohio, has suggested that two of the breakdown products of this vitamin may be associated with an increased risk of heart disease and strokes."   1. Thus, any time someone uses an intermediate indicator like "follow-up utilization of healthcare services", that is a study that begs more questions than it answers.

For example:

  1. Say we develop a Marburg vaccine that reduces mortality from 60-80% to 20%. Survival rate goes up.
  2. Vaccinations against Marburg are thus associated with more follow-up healthcare utilization than the unvaccinated group. (Because they are fucking DEAD).
  3. Intermediate indicators are a fools errand imo
