I know I’m always on about this when cops or other folks kill some agitated person, but for real y’all — we in the ER see people who are angry, threatening, violent, and scary EVERY shift, and we kill zero of them. We don’t shoot them. We don’t choke them to death. Most of the time we talk them down and that’s it. Sometimes we restrain them, but we do it safely.

I have very scary people blow up at me. Often I literally pull up a chair and we talk it out. Please, we can be better.

I’m gonna come out and say it again — if you feel the need to refuse standard of care medical treatment for religious or moral reasons, you don’t belong in medicine. Just fuck off and find a career where vulnerable people relying on you don’t suffer because of your selfishness. I treat nazis. I treat pedophiles. I treat people of all stripes. I’ve treated drunk drivers after I took the radio call to pronounce time of death for their victims.

Joe Biden can do more to try to rein in Israel, but Biden doesn’t run Israel. He isn’t CIC for IDF. He isn’t deciding what hospital to bomb. He isn’t deciding what border crossings to close.

You know who is pushing the Genocide Joe narrative? The fucking Russians and the Israelis. Why is that? Well, it’s an election year, and Trump is a hard right, pro-Israel puppet.

Hi guys, it’s me, your local angry ER doc!

Iowa GOP Senate passed a bill allowing 14 year olds to work night shifts. I have worked countless night shifts. I’ve worked 24 and 36 hour call, and I’ve had to go to lectures or meetings after many of these. I can tell you with certainty that kids who work night shift aren’t learning at school the day after. Night work also increases risk of cardiovascular disease and cancer, among other ill effects. Kids should never work nights. Ever.

With zero surprise whatsoever, we find that the Jacksonville shooter is:

  1. A nazi (literally. He had swastikas on his firearms).
  2. Left a hate manifesto and targeted people of color.
  3. Was a white guy in his 20s.
  4. Used an AR-15 platform rifle (as well as a Glock sidearm).

Jackass couldn’t fit an American mass shooter stereotype better if he tried. We know who does this. We know what they use. Targeted prevention isn’t hard.

It is not “the Jewish people” killing people in Gaza. It is not “Arabs” or “Palestinians” killing people in Israel. The people doing the killing are a tiny number of people, led by a few jackasses. It is, in fact, possible to criticize the people doing the killing, while bearing no ill-will to the ethnic, religious, or national group to which those people belong.

Burning down a refugee camp is not a tragic mishap, especially if you first burned down the refugees original homes, their hospitals, their water pumps and treatment plants, and their fields. No, it is not a mishap. It is an intentional act of violence against people who have lost everything.

If you are a or really anyone working in a and a patient asks you to wear a mask, the correct response is to WEAR A FUCKING MASK. You wouldn’t walk into the OR without a mask. You wouldn’t walk into a bone marrow transplant kid’s room without a mask. Don’t be a dick to people who are high risk for respiratory diseases.

Anthem Blue Cross is now systematically downbilling every level 5 chart my ER physicians group sends them. Our billing people then appeal, and have to appeal multiple times, and eventually months later they accept the appeal on most of them.

This is BCBS stealing from physicians. We did the work. We saw the patients, provided the service for complex and critically ill people, and a company whose CEO makes over $20 million/yr is stealing from us.

As Halloween approaches, it’s time for the annual public health reminder:

  1. Drugs in candy is remarkably rare, and lethal items in candy even moreso.
  2. What does kill kids on Halloween is cars (and the drivers of said cars).

Be careful driving, give little kids light-up or reflective stuff for their costumes, and if you’re gonna have booze, take an Uber or walk. None of us working on Halloween want to see dead Kermit or flat Elsa.

Just to be perfectly clear: -19 can be spread by people who have no symptoms, and usually for at least a week after any initial symptoms. The fact is, if you have COVID-19, you should consider yourself contagious for 10-14 days from the first day you had any symptoms, regardless of how quickly you recovered. It is wise to mask up in public or stay at home during that time.

Lost among indictment news is that the credit rating for the US has been downgraded and is no longer AAA, directly because of Republican threats to shut down the government. This means that it will cost the US, and therefore taxpayers, much more to borrow money. In short, Republicans have made it much more expensive for the US to borrow money, and are wasting billions of taxpayer dollars in the process.

I want each and every one of you who is eligible to vote in the US presidential election in November to stop, think back 4 years, and remember all those videos of guys in body armor beating the everloving hell out of protestors. Remember the unmarked rental vans in Portland? Remember the lines of Feds with M4s on the steps of the Lincoln Memorial? Remember the NG chopper buzzing demonstrators in DC? Remember the guy in Texas driving into the crowd and shooting a guy? That’s the Trump option.

It’s been said by many others already, but the Army and a bunch of defense contractors sponsoring SXSW is just one reason not to participate. The other is that it is a large economic boon to a state that is actively persecuting LGBQT folks, has essentially banned abortion, and is doing everything it can to kill migrants crossing the Rio Grande. Would you want to be seen supporting Hitler’s economy? No? Then don’t support Texas either.

I don’t care if RFK is creeping on OnlyFans. I care that he spread vaccine misinformation for over a decade, leading to numerous outbreaks of vaccine-preventable diseases. He targeted immigrant and african american communities, and in some cases there were outbreaks of diseases like measles within a year or two of his operations. The effects are still being felt.

Here’s a thing to consider: under the ACA (“Obamacare”) insurance cannot exclude pre-existing conditions. A GOP-led Congress and Presidency will destroy that. This is a problem for you and me because:

  1. Nobody has been hiding chronic health problems for 12 years, so we all have pre-existing conditions.
  2. Data mining means insurance companies know how much you drink, what you eat, and what your smartwatch thinks of your heart rhythm.
  3. DNA test services know your genetic risks now.

Once insurance in the US officially and legally can exclude whatever they want again, they’ll use data from your car, phone, smartfridge, home alarm system, and Amazon to undermine coverage. You want them to cover injuries from a car accident? Nope. Your car says you always speed. You get autoimmune hepatitis and your body kills your liver — too bad, Cigna knows you buy alcohol every week and claims that’s the real reason. There will be no running from it.

FYI folks, I’ve admitted 3 folks for COVID in the last two weeks, and had one bounceback after an initial ED visit get admitted. The latter had ground glass opacities on CT of the lungs, which I haven’t seen since Feb 2022 except in one unvaccinated person last September. Maybe it’s a new variant, maybe it’s bad luck, but figured some of y’all might want to know so you can inform your choices in coming weeks.

Donald Trump is suggesting putting people in camps again. It doesn’t really matter who he wants to put in camps. It doesn’t really matter why. What matters is that he, and the men working around him, continue to feel that forcibly placing people in camps is a solution to a problem. These are the same guys who refuse to understand that the constitution very intentionally bars the military from domestic law enforcement. These guys want a dictator.

This is not just about abortion anymore. This about undermining your right to emergency medical care. Once the Republicans undermine EMTALA, hospitals can go back to refusing care for people who can’t pay, or are the wrong religion, or whatever.

Friendly PSA: people can be ardently against killing civilians, bombing hospitals, starving children, flattening schools, and displacing millions of people AND be against a terrorist organization that slaughtered people at a music festival. Believe it or not, those are indeed compatible beliefs. Sometimes people are just against killing innocent people regardless of who those people are. If that’s hard for you, ask yourself what about one of the groups of innocents makes them worth more to you.
